Dog Breath

I have a miniture schnauzer who's breath is horrible!!!!!!!!! I clean/scrap her teeth weekly and brush everyother day. I have tried switching her food, giving her dental chews, even add the minty stuff to her water. Nothing works!!!! Any ideas?????
